Essential Functions:

Provides for patient’s personal hygiene, grooming, and comfort needs, i.e., toileting and elimination, baths, backrubs, shampoos, shaves, bed baths, tub baths or showers, skin and nail care, and oral hygiene.
Promotes a safe environment during care.
Utilizes infection control measures such as hand washing and use of personal protective equipment.
Observes and reports the condition of patient/client to the appropriate supervisor.
Works in a team environment to effectively deliver the care and assistance to patients and staff.
Assists with ambulation, transfers and range of motion exercises.
Performs patient-related procedures such as but not limited to: measuring and documenting intake and output; blood glucose monitoring; urine specimen collections; measurements of vital signs; and weight.
Administers non-medicated enemas, non-sterile dressings, surgical preps, ice packs, heat treatments, sitz and therapeutic baths.
Safely applies and monitors the safe use of physical restraints as directed.
Transports patients and equipment within the facility as necessary.
Completes other assignments as requested and assigned.
May have access to personal health information (“PHI”) necessary to fulfill the above duties and responsibilities. Access to use and ability to disclose PHI is further defined by each organization/department.

Minimum Education & Experience Requirements:

Must be 18 years of age.
High School diploma or equivalent and successful completion of Nursing Assistant course and Nurse Assistant certification as required by state in which employed.
A student RN in the last year of school or an unlicensed RN/LPN/LVN graduate may be eligible for employment as a Nurses Aide as allowable under state law/regulation.
One (1) year experience in a comparable job classification and/or as required by contracted facility.
Knowledge, Skills & Abilities Required:

Pass criminal and abuse background checks, when required by the State(s) in which employed.
Able to communicate with all levels of clinical and non-clinical individuals.
Able to read and interpret technical instructions; use vision and hearing to observe the patient.
Requires hand-eye coordination and fine motor function necessary to assist in the operation of patient care equipment.
Evidence of current CPR/BLS training for infants, children and adults.
Able to provide proof of a valid driver’s license, if applicable.
Able to provide proof of valid auto liability insurance if assignment(s) include driving own vehicle.
Meets applicable health requirements to provide patient care, as appropriate.
Working Conditions & Physical Effort:

Work is normally performed in clinics, schools, hospitals, nursing homes, patient homes or occupational settings.
Work environment involves frequent exposure to communicable diseases, blood borne pathogens and/or potentially infectious or hazardous materials and situations that require following extensive safety precautions and may include the use of protective equipment (e.g. face masks, goggles, latex and or non-latex gloves).

Certified Nursing Assistant

Job Summary: The Nursing Assistant’s primary function is to provide basic patient care under the direction of nursing staff. This position will perform duties such as feed, bathe, dress, groom, change linens, and move residents as directed or needed. As a CMA, this position will administer medications as directed working in our Long Term Care Facility.

Essential Functions / Duties:

Provide direct resident care as assigned, conduct resident rounds daily, and perform nursing care as noted below:
Answer call lights within designated timeframe.
Give care to all residents assigned; make rounds on all residents; check soiled linens and side rails.
Change incontinent residents; wash and dry soiled residents.
Walk ambulatory residents; this includes taking them to the bathroom.
Give tub baths, showers, or bed baths to residents.
Complete oral hygiene and toileting.
Turn and reposition all bed and chair residents.
Place bedpans, linens, washbasins, and soiled linens in proper area.
Complete charting on residents.
Complete food percentage, intake and output, bowel, and bladder records.
Empty and record all urinary drainage units.
Review care plans, update and document nurse aide flow sheets with relevant information; document in nursing assistant notes and nurse aide flow sheets the care and treatment provided to the resident and the resident’s response or lack of response to care provided.
Initiate corrective action as necessary and/or seek assistance from the Charge Nurse; identify special resident problems and report them immediately to the Charge Nurse.
Participate in resident care conferences and other facility meetings as assigned; participate in the development of an individualized plan of resident care for residents as assigned.
Responsible for the safety of residents under his/her supervision.
Participate in facility continuing education programs as required.
Follow facility procedures for admission, discharge, and transfer of residents.
Listen to resident, family, or physician concerns and initiate corrective action and/or report concerns to ADON.
Must be able to work any shift as directed by the ADON.
Comply with federal, state, and company policies, procedures, and regulations.
Operate computers programmed with software needed to record, store, and analyze resident information; operate copy/fax/printer machines to send documents and produce documents.
Perform other related duties as directed by the Administrator.
Shift Specific Duties:

Day
Take residents to the dining room and assist in feeding if assigned.
Serve breakfast and/or lunch trays and assist residents that are unable to feed themselves; remove trays from resident rooms and return them to the dishwashing area; ensure that residents are offered nourishment.
Roll beds down.
Evening
Prepare residents for bed.
Serve evening meal trays and assist residents that are unable to feed themselves; remove trays from resident rooms and return them to the dishwashing area; ensure that residents are offered nourishment.
Take residents to the dining room and assist in feeding if assigned.

Education and Experiences: Prior experience in a similar working environment is preferred, but not a requirement of the position. Home Health Personal Assistant to Individual

Time frame Monday-Thurs (9am-230pm/or 830am-2pm) 5 1/2 hrs day. I am a disabled woman who is an herbalist (botanical medicine), WU alum, intelligent, active, friendly, has many interests and lives a full life. I need help and hands to be an extension of me, when I cannot do things for myself.

Are you willing or experienced in providing personal care? CNA a plus, but not required. Are you a woman seeking extra income or part-time? Prefer someone who is mature, confident to handle task help in community & drive. Must be able to pass a background check/drug screen. Access to reliable transportation is preferred.

I live in my home in a good neighborhood. I am a healthy person without anything infectious or transmissible. Employee will be assisting with personal care, (e,g, body care,bathing, dressing, hair washing etc . Housekeeping tasks such as dishes, trash, vacuuming, laundry etc. I like to cook, but need help with reaching items, preparing, lifting pots, clean up etc. Applicant will also assist me in outdoor things I do such as help me to water my garden, etc.

No total lifting ( I use a transfer board for bed to chair transfers), but applicant needs to be in good physical condition in order to assist with duties I need help with and pushing up/down ramps, navigating in/out of van. Stepping, bending, going up/down stairs for laundry, etc..
As needed, I will either need errands run or have helper drive my lift-equipped van on errands, shopping and any appointments. Must have a valid driver’s license and good driving record. I will show lift operation etc.
