Kaizen – CNA

Description

The Kaizen will provide care to the elders living in a household. Their primary role is to protect, sustain, and nurture the elders by providing assistance with activities of daily living, and meeting other needs as required. The core goal of the work of the Kaizen will be to provide assistance, which promotes a high quality of life and a positive elderhood. The responsibilities of the Kaizen include care of the elder, life enhancement programming and caring for the elder’s environment, including cooking, laundry, and housekeeping. The Kaizen is a member of a team and is responsible for coordinating aspects of the team’s work.

Key Responsibilities/Duties

Report changes in the elder’s condition to the clinical support team per neighborhood change in condition protocols.
Assist elder with personal care functions, including dental and mouth care, bathing, dressing and grooming, bowel and bladder care and other personal and hygiene care.
Meet with the clinical support team as scheduled or needed, to discuss elders needs and progress in identifying and correcting problems attend and contribute to the elders scheduled care plan meetings.
Report accidents and incidents when they occur.
Assist with and/or perform restorative and rehabilitative procedures as outlined by the service agreement.
Observe standard precautions per Brewster Infection Control policy(ies).
Measure and record vital signs, such as temperature, pulse, respirations, weight, height, anatomical dimension/circumference, etc.
Plan and implement life enhancement programming for each individual elder and document appropriately in clinical record.
Record care data on electronic health record (EHR) for each elder at the time of service.
Report any resident abuse or suspected abuse to the mentor immediately after moving elder to safety if necessary.
Maintain confidentiality and privacy of elder care, procedure, and documentation.
Clean and maintain the environment of the household.
Attend and participate in orientation programs, on-going training and educational classes.

What You Will Do

Performs standard testing processes on a variety of equipment and records and analyzes results to determine corrective actions and recommendations for new technologies.

What You Will Need

Minimum H.S. Diploma

Transport Agent/ Extradition Driver

Job Summary:

This position serves as a member of a Transportation Team that executes state and local government transportation contracts. The Transportation Agent is responsible for safely and securely transporting detainees between destinations and ensuring that the vehicles used for transportation are safe and properly maintained. The Transportation Agent ensures that all STS standards and procedures are followed, and metrics are met or exceeded while transporting detainees.

Primary Duties and Responsibilities:

1. Performs vehicle inspections, completes required paperwork to report any damage or defects, and suggests solutions as necessary.

2. Verifies that vehicles have the necessary equipment, and the equipment is in working order before starting a trip. Notifies management if there are any problems before the trip begins.

3. Safely operates vehicles and ensures all safety and legal measures are followed at all times while transporting detainees. This includes all federal, state, municipal and organizational laws, regulations, policies and procedures.

4. Properly logs and tracks movement on logging devices.

5. Reviews all necessary detainee paperwork and reports inaccuracies in a timely manner.

6. Conducts pat-down and strip searches of detainees and ensures that detainees are properly secured prior to leaving a facility and while in a vehicle.

7. Communicates with correctional facilities, jails, and other law enforcement agencies where detainees are being housed to give instructions for their departure and approximate time of arrival.

8. During necessary stops, inspects restrooms or other areas where a detainee may be waiting or housed for possible escape routes.

9. Ensures that all reports are turned into management according to described policies and procedures.

10. Assists team leader and management when unique or unexpected problems arise while transporting detainees. Helps to develop effective and efficient solutions.

11. Performs other duties as assigned.

Qualifications

1. High school diploma or equivalent certification is required.

2. Must be at least twenty-one (21) years of age.

3. A current driver’s license with no record of preventable accidents, suspensions for driving convictions, Driving While Intoxicated, Driving Under the Influence, or more than two (2) moving violations in any type of vehicle for the past three (3) years is required.

4. Pass a company and Department of Transportation required drug and alcohol test as well as a medical certification.

5. Ability to pass a company and federal client background investigation.

6. Ability to own and carry a firearm.

7. Ability and willingness to regularly work an extended time away from home with overnight stays up to one to two weeks at a time.

8. Two (2) years of driving experience in a transportation capacity is preferred.

9. Completion of an approved training program resulting in a certification of satisfactory completion in Corrections, Law Enforcement, or Military is preferred.

10. Three (3) years of Corrections, Law Enforcement, or Military experience is preferred.

General Description:

Communications Specialist I

This is an entry level position with emphasis upon formal and on-the-job training and gaining experience. Incumbent will learn policies, procedures and develop competency in the following areas.

Develops competency in the operation of a highly complex multi-channel system, in order to maintain continuous contact between Troop K Officers, other state, local and federal agencies, and fire and ambulance personnel in a wide geographic area.

Develops competency in accessing, operating, interpreting, disseminating, viewing and responding to numerous types of information quickly that is sent and received from: data entry terminals, the FBI’s National Crime Information Center (NCIC) computer system and its files; the National Law Enforcement Telecommunications System (NLETS); the Kansas Criminal Justice Information System (KCJIS); Kansas Department of Revenue (DOR); Regional Justice Information Service (REJIS) computer system; the SPIDER computer system; El Paso Information Center (EPIC) computer system; and the National Auto Theft Bureau, to exchange information on a worldwide basis and to provide support to a variety of law enforcement functions. These functions could involve operating as many as nine open windows on a minimum of three (3) computer monitors simultaneously. Monitors building alarms on site and the Topeka capitol area.

Develops competency in answering multiple emergency telephone lines to take information pertaining to criminal activity, life threatening situations, Safe School Hotline and vehicle accidents. Coordinates the response of emergency and/or maintenance personnel and equipment to prohibit further threat and destruction of life and property. Collects and disseminates a wide range of information concerning traffic laws, motor vehicle regulations, accidents or complaints from the public in order to provide assistance and emergency call out service. Provides communication coordination for severe weather, war related incidents or other hazards for responding state, federal, local and volunteer agencies.

Maintains detailed logs of individual traffic stops by recording and maintaining minute-to-minute logs of trooper and other state law enforcement activity records by entering information into a computer aided dispatch system. Completes required reports to maintain records for a public internet website containing media releases, follow up on KCJIS/NCIC 2nd hit confirmation requests and maintain statistical information in conformance with federal, state and agency policies. Updates manuals, files, reference materials and logs.
Obtains full access NCIC certification within six (6) months. Develops an expanded knowledge of the NCIC rules and regulations on file usage in order to provide technical support to other agencies.

Develops competency in closely monitoring weather conditions and periods of inclement weather, by requesting and relaying relevant information to Officers, Kansas Department of Transportation and other related agencies. Answers weather inquiries from the public, providing them with the Road Information hotline number, calls for emergency and/or maintenance personnel and equipment, and notifies State Emergency Preparedness and the Governor’s Office in case of disaster associated with the weather.
Provides access control and security of the facility after hours via radio and alarm surveillance. Projects a professional image by wearing and maintaining the Kansas Highway Patrol unit uniform. In emergency situations, may travel to field troop headquarters for dispatch purposes. Performs other reasonably related duties as required by the supervisor.

Certified Medication Aide (CMA)

Lexington Park Health and Rehab is a skilled nursing facility and long term senior care facility offering 24-hour nursing care. Our professional nursing staff works hand in hand with our Rehabilitation, Activities, Dietary and Social Services departments to orchestrate total care for each resident. The Lexington Park team strives to help residents reach their fullest potential.

Job Description: Passes medications and provides direct care to facility residents.

Compensation: $17-18 hr (evening shift gets $2hr added to their base wage and night shift gets $1hr added to their base wage)
