The Charge Nurse will assist the Director of Nursing with management of total resident care for the assigned wing or wings of residents following established facility policy and procedures, federal and state regulations and nursing standards of practice.

Duties:

Major job functions include performing nursing tasks to assure resident needs are met. This includes: monitoring resident needs, condition and care; administering medications and treatments as ordered by the physician; orders and receives medications from the pharmacy; admits and discharges residents; oversees NAR/RCA staff; ensures narcotic policy and procedures are followed; reports changes of condition to RN/Supervisor and physician as delegated; monitors for side effects of medication and reports prn; ensures resident care plans are being followed.

In addition, the Charge Nurse will transcribe and execute physician orders; completes documentation as required; completes fall reporting; reports any bruises, skin tears and abuse/neglect; gives report to nursing staff; follows and upholds Resident Bill of Rights and facility policies and procedures; maintains confidentiality and follows HIPAA requirements, etc. Provides excellent customer service and communication to all residents/families/visitors/MD’s/ coworkers, etc.

Qualifications:

This position requires an RN or LPN licensure with the KS Board of Nursing. Must have current CPR certification. Preference is given to applicants with previous experience in medical, surgical or geriatric nursing. Must have a desire to work with the elderly. Must maintain confidentiality to ensure compliance with all HIPAA regulations.

Job Summary: The Charge Nurse’s primary function is to assess residents’ health problems and needs, develop and implement nursing care plans, and maintain medical records. In addition, this position provides clinical supervision to Nursing Assistants (CNA and CMA) who provide direct resident care.

Essential Functions / Duties:
• Conduct resident rounds daily; report problems to the ADON and initiate corrective action; obtain a report at the start of shift from the nurse being relieved and on that report record sufficient information to implement appropriate follow up action as necessary; provide a report at the end of each shift to the nurse coming on duty, including sufficient information for follow up action if necessary.
• Count narcotics, check emergency kit, and exchange keys.
• Order medications as necessary; administer PRN medications and treatments as necessary; receive medication delivery from pharmacy and check all drugs received and document proof of medications received.
• Document weekly progress notes; update resident care plan when doing weekly progress notes; document daily skilled notes as necessary
• Supervise nursing assist¬ants providing direct resident care; counsel nursing assistants and recommend disci¬plinary action, when necessary, to the ADON; Participate in the employee evaluation process by providing information on the clinical performance of nursing assistants to the ADON.
• Prepare nursing assistants’ assignments and explain all pertinent resident care.
• Supervise the application and removal of all postural supports and restraints.
• Supervise distribution of nourishments.
• Follow staffing guidelines and make nursing assignments according to qualifications of staff and characteris¬tics of the resident population.
• Change linens on all residents as necessary.
• Turn residents as necessary.
• Prepare, administer, and document medications and treat¬ments per the physician’s order and accurately record all care provided.
• Perform comprehensive assessment of each resi¬dent; document comprehensive assessment of each resi¬dent; document resident care provided and resident’s response or lack of response to care provided; report changes in residents’ conditions to a physician, ADON, and responsible party promptly and to take follow up action as necessary.
• Follow facility admission, discharge or transfer procedures and communi¬cate resident admission, discharge or transfer to other departments, physicians, and ancillary service providers.
• Participate in facility education programs and coordinate student activities with the ADON.
• Attend staff meetings, in service classes, and committee meetings as assigned; complete report for facility committees.
• Participate in resident care conferences as as¬signed.
• Supervise response to resident’s call for assist¬ance.
• Responsible for the safety of residents under his/her supervision.
• Listen to resident, family, or physician concerns and initiate corrective action and/or report concerns to ADON.
• Develop an accurate comprehensive care plan based on each resident’s comprehensive assessment; review and revise resident care plans as neces¬sary and as required by regulations; review and revise resident care plan when docu¬menting weekly and daily notes.
• Identify special nursing problems and emergency situations quickly; initiate immediate “life-saving” measures in the absence of a physician.
• Must be able to work any shift as directed by the ADON.
• Comply with federal, state, and company policies, procedures, and regulations.
• Operate computers programmed with software needed to record, store, and analyze resident information; operate copy/fax/printer machines to send documents and produce documents.
• Perform other related duties as directed by the Administrator.

Shift Specific Duties:
Day
o Check breakfast and noon meal trays to ensure proper diet is served and residents are assisted as necessary; supervise the passing of afternoon nourishments.
Evening
o Check dinner trays to ensure proper diet is served and residents are assisted as necessary; supervise the passing of evening nourishments.

Education and Experiences: Prior experience in a similar working environment is preferred, but not a requirement of the position. This position requires an RN (Registered Nurse) or an LPN (licensed practical nurse) in the state of Kansas.

Nurse- Pediatric Full- Time

Our team consists of experienced, compassionate pediatricians and friendly, nurturing staff members who take pride in giving their young patients the personalized pediatric care that they deserve is expanding. We are seeking an individual with the skills and background to compliment our winning team. This position will be responsible for the providing assistance, within their scope of education and experience, in addressing children and adolescent’s illnesses or injury’s, in situations ranging from routine preventative to acute care.Including but not limited, to responsibilities will entail, documenting patient’s symptoms and medical history for the Physician and/or Nurse Practitioner (NP) to review, assisting the Physician or NP during the patient examination and administering certain medical procedures and tests as indicated or instructed. This individual will also periodically be responsible for providing resource information to patient’s and their families on various topics related to the patient’s health and wellness concerns and/or needs.

Minimal Requirements :
• One to two years’ experience in a medical setting, ideally pediatrics, two to three years of experience with additional specialty training in pediatrics for senior consideration. Experience working in an emergency room helpful.
• Knowledge of health care and medical procedures, including terminology.
• Ability to use sound judgment in a fast paced environment, handling a wide variety of situations and people, including emergency or adverse situations.
• Must be able to multi-task and deal with interruptions, changing priorities and time restraints.
• Requires an individual with a high level of patience and compassion, but decisive enough to deal with difficult situations.
• Must enjoy interacting with children and have a gentle and calm enough disposition to be able to deal with children who may be frightened or apprehensive about medical treatment or procedures.
• Must possess cultural awareness and sensitivity.
• Must be able to explain procedures in simple terms so that patient and/or family members understand.
Essential Duties and Responsibilities may include but not be limited to:
• Proactively determines what type of visit patient is being seen for in order to determine set-up needed for visit. Applies education and experience to anticipate what resources may be needed for the patient’s visit (what type of equipment, supplies, shots, etc. that the Physician or NP may need).
• Reviews patient information and records vitals as preparation for the patient’s meeting with the doctor or NP.
• Assists treating physician or NP during examination, treatment and procedures. Helps patients to be comfortable and cooperative with examinations/procedures.
• Collects samples for laboratory tests, administers medications, shots, catheters, removes suture’s, etc. and performs or assists with other treatment or diagnostic procedures as instructed by the doctor. As directed, may perform physical, medical and diagnostic tests or procedures such as eye and hearing exams, ear irrigation, breathing treatments, etc.
• May explain processes, treatment, follow-up needs, etc. to family members and patients in a manner that is understandable to them. Provides education, information and support that may be of interest for that patient or family’s needs.
• Responds to calls from patient or family member, providing information or relaying the inquiry to the physician or NP as appropriate. Schedules appointments as needed.
• Handles or relays information to the medical provider or supervisor regarding issues related to referrals, diagnostic testing centers, hospital admissions/dismissals, etc.
• Records all care information concisely, accurately and completely, in a timely manner, in the appropriate format and on the appropriate forms.Enters/extracts appropriate patient information into and from the computer system. Scans information from the patient’s charts into system.
• Ensures sterile equipment is ready for use by following proper techniques and use of autoclave.
• Ensures Nursing Staff Supervisor is made aware of any concerns from or about patient care issues. Advises Supervisor of equipment issues, supply needs or requisitions, etc.
• Performs other position-related duties as assigned, depending on assignment setting. May float between providers and various clinical services, where qualified and competent.
• Must exhibit a professional image, customer empathy, and exceptional service attitude• Critical-thinking skills are required to assess a patient’s health as well as detect changes in symptoms, health or pain, and recognize when action is necessary.
• Must be compassionate and emphatic to a patient’s needs and, at a more senior level, be able to deal with people in various states of pain, trauma and tragedy.
• Attention to detail and confidentiality is crucial in dealing with patient information, record accurate information, assessing needs, and in administration of medical care including dosage and treatment protocols. Must be organized and know how to prioritize.
• Must be able to work under pressure, remain calm, and handle unusual or urgent circumstances that may be encountered

This position requires excellent communication skills, a patient listener who is not content until they are convinced they are ensured they have a clear understanding of their patients needs and concerns. The ability to communicate in a tactful manner as well as maintain an appropriate demeanor and composure even when faced with an uncomfortable situations is non-negotiable.He or she must be able to “think-on-his/her feet” and respond to instructions from providers and respond to information being given while under pressure. The ability to analyze technical information and provide interpretation so others can understand procedures in a clear manner can not be stressed enough. Excellent command of verbal and written capabilities with an excellent knowledge of medical terminology is highly preferred as well as the ability to read and understand medical product and equipment labels and instructions. The ability to listen and speak effectively with patients, family members and co-workers while concentrating and focusing with constant interruptions is a requirement. In order to provide the most comprehensive care for our young patients the ideal candidate must regularly deal with personal situations involving patients and family members which may include difficult or emergent situations. These situations may significantly increase the stress level of this position and because patients need round-the-clock care, work hours may include days, nights, weekends and holidays periodically be required. This individual must be enthusiastic and eager to work closely with our staff to foster our existing team approach to support, achieve and maintain high quality patient care and cooperation among staff for a cohesive and positive work environment.

JOB OBJECTIVE: To provide medical support, assistance, training, advocacy, evaluation for those individuals served by Day Services/Senior Services and TARC Industries/Employment Services as well as their TARC staff.

IMMEDIATE SUPERVISOR: Day Services/Senior Services Director

RESPONSIBILITIES:

ASSESSMENT AND CARE
*Provide assessment and care:
Provide weekly health checks as identified by the persons supported Program Coordinator.
Provide nursing care when necessary to ensure the welfare of those supported.
Provide emergency medical supports.
Provide environmental checks to ensure universal precautions are routinely followed.
Provide daily medical checks for all of those supported in Day and Senior Services.
Attend PCSP and team meetings for those persons with significant medical needs.
Provide monthly observation of medication administration and med logs.
Follow up on all recommendations.
Inform Day Services Director when medical needs of persons supported are unmet or care/treatment is being compromised.

*STAFF TRAINING
Develop written curriculum and provide staff with in-service training in the areas of:
diabetes
universal precautions
medication
medication administration and documentation
suction machine
nebulizer
blood pressure
pressure sores
tube feeding and irrigation
infection and disease control
Hepatitis B
Other related health topics as they arise.
Work with new and experienced staff and teach methods to meet the medical needs to include proper transfer techniques (two person transfer/Hoyer Lift Transfer), universal precaution, etc of those we serve as outlined on the persons supported’s PCSP.
Work with the DS/SS Director and Training Coordinator in providing in-service training opportunities.

*EQUIPMENT AND SUPPLIES
*Maintain all medical equipment and ensure all medical equipment is in working condition.
Develop and maintain equipment logs.
Provide input to the Day Services/Senior Services Director for needed materials and equipment necessary for meeting persons supported’s medical needs, i.e. sanitary gloves, individual medical supplies, etc.
Provide input to the Day Services/Senior Services Director re: medical equipment and supplies needed for the Division’s annual budget.

DOCUMENTATION
*Document any and all medical assessment, evaluation, and Range of Motion and Aquatic Therapy notes, follow up, training provided, and consultation and maintain accurate records in compliance with state and federal regulations and guidelines.
Perform monthly review of medication logs.
Perform monthly review of logs for any medical procedure performed.
Provide monthly reports and data, as requested.

STAFF PERFORMANCE
Perform direct nursing and therapeutic procedures utilized by licensed nursing staff.
*Train staff in the administration of medication to persons supported, Range of Motion exercise to include Aquatic exercises, the training of persons supported to self-administer medication (where appropriate), and the provision of treatments, as prescribed by a licensed physician.
Report any medication errors immediately to the Day Services/Senior Services Director.
Provide on-going follow-up to ensure staff are following appropriate procedures.
Provide supervision and direction over the Aquatic Therapy Program.
Supervise and train the Restorative Aide and Aquatic Specialist.

COMMUNITY AND TEAM SUPPORT
*Represent TARC in the community by working in cooperation with medical professionals, consultants, residential providers and guardians/families.
Develop and maintain an effective, professional relationship with team, parents, guardians, colleagues, and TARC staff to ensure effective communication. *Actively participate on the Behavior Management Committee.
Work with Day Services/Senior Services Team to continue the advancement and level of service delivery provided by Day Services/Senior Services.
Actively participate in Day Services team meetings through effective communication, openness to suggestions and new ideas.

OTHER DUTIES AS ASSIGNED
Assist in the upkeep, organization and sanitation of the program environments and any materials/equipment utilized.
Attend appropriate in-services and present information/resource material available to Day Services/Senior Services staff.
Maintain awareness and practice TARC, Inc. policies and procedures.
Exercise TARC, Inc. philosophy and Code of Ethics.
DS/SS Representative for the Safety Committee
Coordinate and perform TARC’s TB clinic for TARC employees

EDUCATION: Registered Nurse (RN) License for the State of Kansas with a Bachelor’s degree in nursing. Emphasis in psychiatric nursing preferred.

EXPERIENCE AND TRAINING: Registered Nurse with five years experience working with persons with developmental disabilities.
