Job Summary: The Nursing Assistant’s primary function is to provide basic patient care under the direction of nursing staff. This position will perform duties such as feed, bathe, dress, groom, change linens, and move residents as directed or needed. As a CMA, this position will administer medications as directed.

Essential Functions / Duties:
• Provide direct resident care as assigned, conduct resident rounds daily, and perform nursing care as noted below:
o Answer call lights within designated timeframe.
o Give care to all residents assigned; make rounds on all residents; check soiled linens and side rails.
o Change incontinent residents; wash and dry soiled residents.
o Walk ambulatory residents; this includes taking them to the bathroom.
o Give tub baths, showers, or bed baths to residents.
o Complete oral hygiene and toileting.
o Turn and reposition all bed and chair residents.
o Place bedpans, linens, washbasins, and soiled linens in proper area.
o Complete charting on residents.
• Complete food percentage, intake and output, bowel, and bladder records.
• Empty and record all urinary drainage units.
• Review care plans, update and document nurse aide flow sheets with relevant information; document in nursing assistant notes and nurse aide flow sheets the care and treatment provided to the resident and the resident’s response or lack of response to care provided.
• Initiate corrective action as necessary and/or seek assistance from the Charge Nurse; identify special resident problems and report them immediately to the Charge Nurse.
• Participate in resident care conferences and other facility meetings as assigned; participate in the development of an individualized plan of resident care for residents as assigned.
• Responsible for the safety of residents under his/her supervision.
• Participate in facility continuing education programs as required.
• Follow facility procedures for admission, discharge, and transfer of residents.
• Listen to resident, family, or physician concerns and initiate corrective action and/or report concerns to ADON.

• Day
o Take residents to the dining room and assist in feeding if assigned.
o Serve breakfast and/or lunch trays and assist residents that are unable to feed themselves; remove trays from resident rooms and return them to the dishwashing area; ensure that residents are offered nourishment.
o Roll beds down.
• Evening
o Take residents to the dining room and assist in feeding if assigned.
o Serve evening meal trays and assist residents that are unable to feed themselves; remove trays from resident rooms and return them to the dishwashing area; ensure that residents are offered nourishment.
o Prepare residents for bed.

Employment Associate Prod/Community

JOB OBJECTIVE: Work under the guidance of the TIES Assistant Director to provide individualized employment training and services to adults with developmental and related disabilities. Work effectively as a team member to meet persons supported employment needs, as well as the production needs within TARC Industries. Provide training and support services at community work sites.

IMMEDATE SUPERVISOR: TIES Assistant Director

PRIMARY RESPONSIBILITIES:

I. PERSONS SUPPORTED RESPONSIBILITIES
A. *Provide individualized support to individuals in order to accomplish their personal work goals.
B. *Implement/Support objectives as identified in Person Centered Support Plans.
C. *Utilize reasonable accommodations to achieve individual’s maximum independence and utilization of job skills, by all individuals.
D. Ensure all Human and Legal Rights of individuals are respected and preserved.
E. Ensure communication with all teams involved to generate opportunities and ideas to support goals, dreams, quality of life, and independence.
F. Develop and maintain an effective, working relationship with team, guardians, community job site employers and colleagues to ensure effective communication regarding satisfactory delivery of services.
G. Provide persons supported and community with positive support models.

II. ASSISTIVE TECHNOLOGY
A. Identify needs for speech therapy evaluations and for training on new communication devices.
B. Identify needs for overlays and make referrals to Assistive Technology Department.
C. Initiate referrals for repair/recharging of communication devices.
D. Track charging schedules so devices are always available for use.
E. Ensure follow through of persons supported’s personal communication goals while at TARC.
F. Initiate referrals to Assistive Technology Department when needed in regards to accommodating individuals in performing their daily duties.

III. DOCUMENTATION
A. *Maintain accurate, thorough and efficient documentation in accordance with TARC Policies and Procedures.
B. Maintain supported employee work file and progress reporting in a current and accurate manner.
C. *Conduct job and task analysis to determine basic elements of job(s) to be learned and performed by person supported.
D. *Provide accurate timecards of persons supported for funding purposes.

IV. PRODUCTION
A. Ensure quality and production standards are maintained.
B. Ensure safety rules and regulations are adhered to.
C. *Assist Production Manager with contract layout/setup and time studies as needed.
D. Work as an effective team member to ensure work orders, quality standards and completion dates are adhered to.

V. COMMUNITY WORK SITES
A. Educate persons supported on community employer rules, expectations, policies and operating procedures.
B. Provide transportation training as required.
C. Work with community employer to identify any modification to job description, restructuring of job duties or reasonable accommodations.
D. Obtain feedback from community employer and implement plan to address areas of concern and report any changes or needs to Employment Program Coordinator.
E. Communicate with Program Coordinator on a regular basis and provide feedback to team on progress, areas of concern, highlights and achievements.
F. Work with Program Coordinator to implement fading plan by gradually turning supervision and support to community employer.
G. Provide ongoing support, as needed.

A. Requires creativity to analyze situations and advocate participation in the development and implementation of new opportunities.
B. Must be able to handle diverse work situations on a daily basis.

Must be patient and creative. Math skills should reflect the ability to accurately count and record data.

EDUCATION: High School Diploma or equivalent.

EXPERIENCE AND TRAINING: Two years working with individuals with developmental disabilities preferred; and/or appropriate coursework in the MR/DD field. Production, housekeeping and janitorial skills experience helpful.

Direct Support Professional

JOB OBJECTIVE: Develop and maintain ethically appropriate relationships with the people you serve. Offer choices that support and increase independence, integration, inclusion, and self advocacy. Assist in the coordination and implementation of daily activities, including but not limited to, computer usage, crafts, hobbies, leisure activities, specialized services, volunteer opportunities, and participation in community based services. Assist in the assessment, response, and documentation of medical issues ranging from minor bumps and bruises to major medical emergencies. Provide dignity, respect, and quality of living for people experiencing end of life issues. Assist in the maintenance and enhancement of Life Skills training and provide the highest quality, most person supported driven services that will lead to growth and self-advocacy.

IMMEDIATE SUPERVISOR: Day Services/Senior Services Assistant Director

RESPONSIBILITIES:

I. PERSON CENTERED SERVICES/PROGRAMS
A. *Implement/support objectives and goals as identified in the person supported’s Person Centered Support Plan (PCSP). Document and follow up, as needed.
B. Serve as a positive role model for all persons supported.
C. Submit PCSP assessment material and all other requested data collection for conferences, team meetings, and reviews to the Program Coordinator.
D. Utilize reasonable accommodations to maximize persons supported’s independence and quality of their life.
E. Ensure all human and legal rights of persons supported are preserved and respected.
F. *Provide an enthusiastic environment which promotes and encourages self-discovery, growth, and learning opportunities.
G. Develop/adapt environments to promote persons supported’s full inclusion and independence.
H. Creatively assist in the planning and organization of daily, weekly and monthly activity schedules.
I. Assist persons supported in obtaining their maximum potential in the community to include volunteer opportunities.
J. Collaborate with the Program Coordinator and Ds/SS team to create, plan, and implement additional opportunities in the community.
K. *Maintain and enhance persons supported personal hygiene, social interaction and routine daily living skills through ongoing Life Skills training.
L. Ensure all specialized services are followed through as trained by the Registered Nurse and Physical Therapist consultant.
M. Adhere to and be knowledgeable of all safety rules and regulations pertaining to transportation, building, and persons supported and staff well being.
N. Adhere to program schedule keeping in mind the routine may change due to health/aging issues.
O. Communicate with the Program Coordinator prior to contacting parents/guardians regarding issues, needs and/or concerns.
P. Attend staff meetings and PCSP conferences to provide input and insights to programs, persons supported’s involvement and development, and activity updates.
Q. *Provide rotation of attention to persons supported.
R. Assist persons supported to maintain mental and physical clarity by providing stimulating activities which adds to the quality of their lives.
S. Allow persons supported’s input into the activities they participate in as well as provide activities they may not have experienced yet which they may enjoy based on their interests.

II. COMMUNICATION/ASSISTIVE TECHNOLOGY
A. Identify needs for overlays and make referral to the Program Coordinator and the Educational Coordinator.

C. Attend appropriate in-services, and present information/resource material available to Day Services/Senior Services staff
D. Maintain awareness and practice agency policies and procedures.
E. Exercise agency philosophy and Employee Code of Ethics.
F. Provide emotional support to persons supported and co-workers as they experience loss and change in relation to the aging process.
