JOB OBJECTIVE: Develop and maintain ethically appropriate relationships with the people you serve. Offer choices that support and increase independence, integration, inclusion, and self advocacy. Assist in the coordination and implementation of daily activities, including but not limited to, computer usage, crafts, hobbies, leisure activities, specialized services, volunteer opportunities, and participation in community based services. Assist in the assessment, response, and documentation of medical issues ranging from minor bumps and bruises to major medical emergencies. Provide dignity, respect, and quality of living for people experiencing end of life issues. Assist in the maintenance and enhancement of Life Skills training and provide the highest quality, most person supported driven services that will lead to growth and self-advocacy.
IMMEDIATE SUPERVISOR: Day Services/Senior Services Assistant Director
I. PERSON CENTERED SERVICES/PROGRAMS
A. *Implement/support objectives and goals as identified in the person supported’s Person Centered Support Plan (PCSP). Document and follow up, as needed.
B. Serve as a positive role model for all persons supported.
C. Submit PCSP assessment material and all other requested data collection for conferences, team meetings, and reviews to the Program Coordinator.
D. Utilize reasonable accommodations to maximize persons supported’s independence and quality of their life.
E. Ensure all human and legal rights of persons supported are preserved and respected.
F. *Provide an enthusiastic environment which promotes and encourages self-discovery, growth, and learning opportunities.
G. Develop/adapt environments to promote persons supported’s full inclusion and independence.
H. Creatively assist in the planning and organization of daily, weekly and monthly activity schedules.
I. Assist persons supported in obtaining their maximum potential in the community to include volunteer opportunities.
J. Collaborate with the Program Coordinator and Ds/SS team to create, plan, and implement additional opportunities in the community.
K. *Maintain and enhance persons supported personal hygiene, social interaction and routine daily living skills through ongoing Life Skills training.
L. Ensure all specialized services are followed through as trained by the Registered Nurse and Physical Therapist consultant.
M. Adhere to and be knowledgeable of all safety rules and regulations pertaining to transportation, building, and persons supported and staff well being.
N. Adhere to program schedule keeping in mind the routine may change due to health/aging issues.
O. Communicate with the Program Coordinator prior to contacting parents/guardians regarding issues, needs and/or concerns.
P. Attend staff meetings and PCSP conferences to provide input and insights to programs, persons supported’s involvement and development, and activity updates.
Q. *Provide rotation of attention to persons supported.
R. Assist persons supported to maintain mental and physical clarity by providing stimulating activities which adds to the quality of their lives.
S. Allow persons supported’s input into the activities they participate in as well as provide activities they may not have experienced yet which they may enjoy based on their interests.
II. COMMUNICATION/ASSISTIVE TECHNOLOGY
A. Identify needs for overlays and make referral to the Program Coordinator and the Educational Coordinator.
B. Initiate referrals for repairs and recharging of communication devices.
C. Deliver communication devices to the Assistive Technology Department when repair and recharging is needed. Track charging schedule so devices are always charged and ready for persons supported to use.
D. Be responsible for follow through of persons supported’s personal communication goals when supported at TARC, Inc.
E. Be aware of communication needs and challenges for individuals as they experience aging issues, behavioral and/or medical challenges.
A. *Maintain accurate, thorough and efficient documentation in accordance with TARC, Inc. Policies and Procedures:
IV. TEAM CONCEPT
A. Convey a positive image of TARC, Inc. and its program to the community.
B. Participate in team meetings to identify/advocate/support persons supported’s wants, needs, goals and dreams.
C. Continue communication with persons supported and support team to generate opportunities and ideas, which will support goals and dreams to persons supported’s satisfaction and independence.
D. *Develop and maintain an effective, professional relationship with team, parents, guardians, colleagues to ensure effective communication regarding universal service delivery and satisfaction of services.
E. *Actively participate in Day Services/Senior Services team through effective communication, open to suggestions, flexibility, and new ideas.
V. OTHER DUTIES
A. Provide input for needed materials and equipment necessary for meeting persons supported needs.
B. Assist in the upkeep, organization and sanitation of the program environment and any materials/equipment utilized.
C. Attend appropriate in-services, and present information/resource material available to Day Services/Senior Services staff
D. Maintain awareness and practice agency policies and procedures.
E. Exercise agency philosophy and Employee Code of Ethics.
F. Provide emotional support to persons supported and co-workers as they experience loss and change in relation to the aging process.

QUALIFICATIONS: Person of good character who is mentally and physically able to conduct oneself in a responsible manner in carrying out the job for which they are employed.
EDUCATION: High school diploma or equivalent required. One year of post-high school training preferred.
EXPERIENCE AND TRAINING: One year experience working with individuals with disabilities, elderly population and/or the completion of coursework in the human services field preferred.
